are you seeking to have your child baptized at our lady of the mountains?The Parents must fulfill the following
- Provide a copy of the child's birth certificate.
- Complete the Baptism Information form.
- Complete the Publicity Release for Adults & Minors for their child's picture to appear in the Bulletin, Parish Newsletter, and on our website.
- Parents will need to complete a Baptism Preparation class at Our Lady of the Mountains or at their home parish. Baptismal preparation classes can be scheduled on the second Monday of each month from 4:00 to 6:00 p.m.
- If parents belong to a parish other than Our Lady of the Mountains they will have to provide a letter of permission from their pastor to have their child baptized at Our Lady of the Mountains.
The Godparent/Sponsor must fulfill the following- Confirmed and at least 16 years old.
- Living a chaste life as a single person or be married in a valid Catholic marriage.
- Actively living their faith, attend Mass and confession regularly.
- Must be able to produce a proof of Baptism (stamped with a seal) to be delivered to the Parish office.
- Only one Catholic Godparent is required. A non-Catholic may not be a Godparent, but may be a witness.
- Complete the Godparent/Sponsor Eligibility form.
- Complete a baptism preparation class at Our Lady of the Mountains or at their home parish and provide proof of the same. Godparent/sponsor eligibility classes can be scheduled on the second Monday of each month from 4:00 to 6:00 p.m.
